Dear Parents,

We are now beginning to pre-register students for the 2011-2012 school years. We have work to keep rates as
affordable with options; and are sure that we are more affordable than other Catholic or Private Schools.

Attached you will find forms that must be completed in order to register your child/children at Our Mother of
Mercy School. To register for Pre-K 3, Pre-K 4 and Kindergarten, your child must be three, four, or five by
September 1st in the current school year.

The following documents will be required FOR ALL STUDENTS (this includes sibling attending for the first time).
PLEASE ENCLOSE A COPY OF EACH DOCUMENT REQUIRED WHEN YOU RETURN YOUR REGISTRATION PACKET.

BIRTH CERTIFICATE –STATE ISSUED (OFFICIAL WITH SEAL)
SOCIAL SECURITY CARD
UP-TO-DATE IMMUNIZATION RECORDS FROM YOUR PHYSICIAN OR HEALTH DEPARTMENT.
BAPTISMAL CERTIFICATE (IF CATHOLIC)
FIRST COMMUNION CERTIFICATE (IF CATHOLIC)

YOUR REGISTRATION IS NOT COMPLETE UNLESS ALL FORMS ARE RETURNED, INCLUDING THOSE ITEMS LISTED ABOVE. ALL OUTSTANDING TUITION, FEES, ETC FOR THE SCHOOL YEAR 2010-2011 MUST BE PAID BEFORE REGISTRATION WILL BE ACCEPTED.

Tuition for the school year will be broken down to 10 payments beginning in July 1st and ending April 1st. All tuition is due on the 1st of the month, and becomes late on the fifth incurring a $25 late fee. Registration for School Year 2011-2012 will be $75.00 payable at the time of registration. An annual Maintenance/Book Fee of $300 is assessed per student also. Both Fees must be paid by May 15, 2011.
